The Breast Lift Procedure

All breast lifts are performed under general anesthesia. Incisions used in breast lifts can vary depending on the type of breast lift and the severity of laxity. Dr. Yang will discuss this with you during your consultation before your surgery.  At a minimum, a circular incision will be made in the skin surrounding your nipple.

Some patients may also need an incision that extends from the bottom of their areola (the colored area around their nipple) to the fold beneath their breast. In some cases, an incision may also be needed underneath their breasts as well. Dr. Yang will remove and tighten the excess skin, reshaping and lifting the breasts without affecting the chest muscles. Once the desired chest placement and contour are achieved, Dr. Yang will close the incisions.

Most patients take a week off from work to recover. Breast lift surgery requires time to heal. This is something that patients need to understand before undergoing a breast lift. It takes about one month to return to light exercise before you are healed. You will need to wait two to three months before engaging in competitive sports.

These are two distinct surgeries that address different concerns, though they can sometimes be combined for the best outcome.

A breast augmentation primarily increases the size and enhances the fullness of your breasts. It involves placing saline or silicone implants to add volume, improve shape, correct asymmetry, or restore lost fullness (often after pregnancy, weight loss, or simply due to genetics). If your main goal is to have larger, rounder, or more proportionate breasts, and you’re generally happy with their position and degree of firmness, augmentation alone is often the right choice.

A breast lift, on the other hand, focuses on elevating and reshaping sagging breasts without significantly changing their size. It removes excess skin, tightens the underlying tissue, and repositions the nipple-areola complex higher on the breast mound for a perkier, more youthful appearance. This is ideal if your breasts have drooped due to aging, pregnancy/breastfeeding, gravity, or significant weight fluctuations, even if you’re content with your current cup size.

Many patients ask how to tell which one they truly need. Here are some key self-assessment points I discuss in consultations:

If your breasts feel too small, deflated (especially in the upper pole), or disproportionate to your frame, and they sit at a reasonable height on your chest → breast augmentation is likely the primary procedure.


If your nipples point downward, fall below the inframammary fold (the crease under your breast), or there’s noticeable sagging/”pencil test” (a pencil stays tucked under the breast without falling), and excess skin makes them appear elongated → a breast lift is usually necessary to correct the position and shape.

If you want both more volume/fullness and a higher, firmer position → a combined breast augmentation with lift (augmentation mastopexy) is frequently the best option. This is very common after pregnancy or major body changes.

Breast ptosis (sagging) grading, skin elasticity, nipple position, and your specific aesthetic goals all play a role. Patients want the least noticeable scars possible while achieving a perky, youthful breast appearance. Scars from breast lift surgery typically fade significantly over 12–18 months with proper care (silicone sheets, massage, and sun avoidance), often becoming subtle lines. 

The breast lift with the least scars depends on your degree of sagging (ptosis):

For minimal sagging, the crescent lift uses the smallest incision—a half-moon along the upper areola border. This results in the least scarring, often virtually invisible as it blends into the areola’s edge. It’s commonly combined with breast augmentation.

More extensive options like the vertical (“lollipop”) or anchor (inverted-T) lifts offer greater reshaping for moderate to severe sagging but involve additional scars (vertical down the breast or along the crease).

The key is matching the technique to your anatomy for optimal results with the minimal scarring breast lift approach. I always prioritize careful technique and precise closure for fine-line healing.

A breast lift alone does not significantly change your actual breast volume—it reshapes and repositions your existing tissue by removing excess skin and tightening the underlying structure for a perkier, more youthful contour. In most cases, your breasts will not look larger after a breast lift without implants, as no additional volume is added (unlike with breast augmentation).

However, many patients notice their breasts appear fuller and rounder in the upper pole due to the lifted position and restored projection—this can create an illusion of subtle enhancement using your own tissue. That said, breasts often appear slightly smaller or more compact post-surgery. This happens because excess stretched skin is removed.

Results vary based on your starting anatomy—if you have good natural volume, the lift can make breasts look shapelier and even perkier/fuller without implants.

If deflation is significant, you might consider combining a breast lift with implants (augmentation mastopexy) for added size and fullness.

The best way to predict your outcome is an in-person exam, during which I assess your ptosis, skin elasticity, and tissue distribution.

Crescent breast lift: This is the most minimal technique. It uses a small, half-moon (crescent-shaped) incision along the upper edge of the areola only. Excess skin is removed from above the nipple-areola complex to provide a subtle lift (typically 1–2 cm nipple elevation). It’s ideal for very mild sagging or minor nipple asymmetry, often combined with breast augmentation for the best results. Scarring is the least noticeable—a thin line that blends into the areola’s natural border.

Vertical breast lift (lollipop lift): This offers more comprehensive correction. It includes a circular incision around the entire areola plus a straight vertical incision down from the areola to the breast crease (forming a “lollipop” shape). It removes more excess skin, tightens tissue, reshapes the breast for better projection and contour, and repositions the nipple higher—making it suitable for moderate sagging. It provides greater lift and a more youthful, conical shape than the crescent, but it also leaves an additional vertical scar (which fades well over time and is usually hidden under clothing).

In short, the crescent lift is for subtle, minimal changes with almost invisible scarring, while the vertical lift delivers more significant lifting and reshaping for moderate droop, with modestly more scarring as the trade-off. The right choice depends on your anatomy—ptosis grade, skin elasticity, breast volume, and goals (e.g., whether you want more fullness or improved shape). An in-person exam and measurements are essential to recommend the optimal breast lift technique that balances results with minimal scars.
